Kathy Gallagher Summer Scholarship

DEADLINE: MAY 7, 2013.

Current players who have been members of any ESYO ensemble for at least one full season may apply for a Kathy Gallagher Summer Scholarship to help pay for summer music camp, private lessons, or the purchase or repair of an instrument. Graduating seniors and players not planning to return to ESYO for the next season are not eligible.  These awards are based on financial need and the player’s general contribution to ESYO.  Applications are available in April; award recipients are notified in June.

ESYO reserves the right to request return of scholarship funds if award recipients do not continue to play with the ESYO organization.

To be considered for a scholarship, please complete and submit the form below.  You must also provide us with a copy of your 2012 Federal Income Tax Return.  Please mail it to:  Scholarship Committee, Empire State Youth Orchestras at Proctors Theatre, 432 State St., Schenectady, NY 12305.

Kathy Gallagher Scholarship Fund Application for Summer 2013

  • List music programs (summer, school-related, other) in which you have participated, along with the date(s) of your participation. Ex: The College of Saint Rose Summer Jazz Program, June 25 - August 2, 2012.

  • 2012 Adjusted Gross Income from FATHER'S Federal Return if filed separately
  • 2012 Adjusted Gross Income from MOTHER'S Federal Return if filed separately
  • We urge you to describe any other financial considerations that might help us make decisions about this scholarship.
  • PLEASE REVIEW APPLICATION BEFORE SUBMITTING.
  • BE SURE TO SEND A COPY OF THE FIRST 2 PAGES OF YOUR 2012 FEDERAL INCOME TAX RETURN TO: Scholarship Committee, Empire State Youth Orchestras at Proctors Theatre, 432 State St., Schenectady, NY 12305. DUE DATE: MAY 7, 2013.
  • NOTE: Applications are NOT complete until we receive the copy of the first 2 pages of the tax return.
  • You may also send your tax return via e-mail by sending .PDF attachment of your form to esyosec@esyo.org. If you have any questions regarding sending your return via e-mail, please call ESYO at (518) 382-7581.

 

Careers in Music Scholarship

The Careers in Music Scholarship is awarded at the end of each ESYO season to a graduating senior from any ESYO group who best exemplifies the goals and standards of his/her ensemble, serves as a role model to his/her peers, has made outstanding musical contributions, and plans a career in music. It is a $500. award.

  • DEADLINE: All applications and teacher recommendations must be submitted by MAY 7, 2013.
  • REQUIREMENTS:
    • Musician must complete the application below.
    • Musician must ask his/her music teacher to complete a Teacher Recommendation Form (below – click here). Your application for the Careers in Music Scholarship will not be considered complete until we receive the recommendation from your music teacher.

Careers in Music Scholarship Application

  • Please enter the name of your school. For home schools, please enter "home."
  • For public schools, please enter the name of the school district. For private schools, please enter "private." For home schools, please enter "home."
  • For public and private schools, please enter the name of your school music teacher. For home schools, please enter "n/a."
  • For public and private schools, please enter your school music teacher's school e-mail address. For home school, please enter "home school."
  • Please list the instrument(s) you play followed by the number of years of study on the instrument(s). Example: trumpet - 5; piano - 3.
  • Please list all musical groups (school-related or others but not including ESYO groups) and the grade level in which you participated in them during high school. Example: Orchestra - grades 9, 10, 11, 12; Jazz Band - grades 10, 11. If home schooled, please enter grade equivalent.
  • Please list all music camps and programs (not including ESYO) and the grade level in which you participated in them during high school. Example: Eastman Summer Jazz - grade 11; SOS - grade 12. If home schooled, please enter grade equivalent.
  • Please answer the following essay questions. Please be concise but thorough.

Careers in Music Scholarship - Teacher Recommendation Form

Your music student has applied for ESYO's Careers in Music scholarship. This $500 award is given to a graduating senior who best demonstrates the ESYO spirit at school and in the community and who plans on making a career in music (including performance, education, therapy, business, etc.). Please complete the following recommendation form and submit it to us by May 7, 2013. Thank you.
  • Public/private school teachers, please enter your position. Private teachers (of home schooled students only) please enter, "private."
  • Public / private school music teachers - please enter name of school; Private teachers (for home schooled students only) please enter, "private."
